Intent would appear to be ' entered false information' possibly 'enter misinformation'. I'd go for 'false information' being directly to the point these people lie and create false records. You have done a lot to uncover this.

Thank you for your efforts in holding these perpetrators accountable. Was I the only person who was horrified to read the HCQ study where they purposely overdosed the drug (by multiples of safe dosage) to very very ill people in a hospital, effectively murdering them to prove a point and keep HCQ from safely and effectively being used for early treatment? I There won’t be a prosecutor with any stones to hold the monsters accountable, but am so relieved to see complaints like this being brought. Hero!
